Passport
- Original passport issued within 10 years
- Valid for 6 months beyond the last intended date of departure from Poland
- Should contain at least 2 blank visa pages
- Should have no alteration or handwritten amendments in the data pages
Photograph
- Three recent colored passport size photographs (35 mm wide by 45 mm high without border) and taken within the last 3 months
- The facial image must be between 25 mm and 35 mm from chin to crown
- Taken against a plain white background with matt or semi-matt finish
- Please note that scanned or unclear photographs will not be accepted
Accommodation
- Applicant must provide one of the accommodation proof :
- Holding confirmed Hotel Voucher
- Cruise Confirmation Voucher
- In case staying with friend’s or relative’s place then invitation letter with invitee Id proof, passport or residence card copy along with their financial documents (email with colored scan copies will work)
Flight Tickets
- Confirmed flight reservation (India to onward and Return)
Itinerary
- Day wise itinerary which gives information about your travel plans on plain A4 size paper or else if booked through travel agent then on their letterhead
Travel Insurance
- Travel insurance is mandatory with coverage of minimum 30,000 Euros or 50,000 USD / person
- All expenses which include repatriation for medical reasons, urgent medical attention or emergency medical treatment in hospital
- The insurance should cover the entire stay in the territory of Schengen
- Applicants applying for a period of 6 months or one-year multiple-entry visas are not entitled to provide 180/365 day coverage. Submitting an insurance policy that covers the first journey is sufficient
Proof of sufficient funds for undertaking travel
- Applicant needs to provide last 6 months bank statement signed and attested by a bank official in case the applicant is employed else need to provide bank statement of the person sponsoring your trip
- All bank statements must be dated within the last 1 week
Income Tax Returns
- Income tax return assessment for the last 3 years in case the applicant is employed else need to provide income tax return of the person sponsoring your trip
Covering letter
- A Covering letter from the Applicant on A4 Size Paper (if Employed) or else on Business Letter Head (if Self Employed)
Employment letter
- Employment letter on official company letterhead, stamped and dated; containing contact information (address, email, telephone number) as well as name, position and signature of the countersigning officer
- The letter must state the name and position of the applicant, salary, years of employment and approval of absence/position after returning
NOC
- NOC from School / Institute along with Xerox of ID card (in case of student / minor)
GST copy
- If Self Employed then applicant need to provide a GST Copy
Leave letter
- Leave letter from the company along with a copy of Employment ID Card (if employed)
Biometric
- Indian citizens applying for a Schengen visa for the first time will have to appear in person in order to provide biometric data

- Previously submitted biometric data (after 2nd November 2015) is valid for travel to any Schengen country for 59 months from the date of submission

If I am in possession of valid Multiple entry Schengen visa on my old passport, then whether can I travel by carrying my both passports?
This will vary from Country to Country, some Schengen countries will allow Applicant to travel by carrying both Old & New passport, if visa is valid on Old passport & some countries will require applicant to transfer the visa from Old passport to New passport. Is it necessary to have Personal Appearance for Visa submission?
From 02nd November 2015, Schengen Countries have started Biometrics process for visa submission, so every applicant has to go personally to VFS (Visa Facilitation Center), a body appointed by Consulates OR in some countries directly he has to visit Consulate/Embassy for completing Biometrics procedure i.e. to capture Finger Printing, palm printing, Live photo.

Is biometrics being one Time process?
As per the Standard Rule set by Schengen authorities, Biometrics is valid for 59 months & it is applicable for all Schengen countries & it can be easily identified from VIS remark on your visa issued after your First biometrics done, so if you have completed your Biometrics earlier for Schengen visa after 02nd November 2015 then you need not do it again for your subsequent Schengen visa application.But here again few countries will require applicant to come every time for Biometrics whenever they apply irrespective of criteria of 59 months OR few countries follow 59 months’ criteria but may call passenger for Biometrics again if there is some problem in Biometrics details captured in Visa Information System. How many days before can I apply for my Schengen Visa?
Maximum 3 months before your travel date, Applicant can apply for Schengen visa, before that it will not be possible to apply for Visa.
If Long Term Multiple entry visa is valid on my current passport before Biometrics process started?
In this case, you can very well travel on that visa even though it was issued before biometrics process started.
